Medical Consent

Each time your child attends a trip or visit, it is important that we have the most up to date information regarding any medical conditions that your child may have. For this reason, we do not use historical medical records for trips. Parents are encouraged to download a copy of the form and save to your desktop, which then can be updated for any forthcoming trips and visits.

Please may we request that copies are printed and sent into the Academy, with each trip, as email attachments are subject to data protection.
